Funeral Service Will

Complete this Funeral Service Plan to record your wishes for your Christian funeral and burial, including preferred Scripture readings, hymns, service arrangements, and other instructions. This plan will help your family and pastor prepare for your funeral while also giving you an opportunity to leave a final testimony of your faith and hope in Jesus Christ.

Knowing full well that the wages of sin is death, I confess that Jesus Christ died for the forgiveness of all my sins. This forgiveness and grace I neither deserve nor merit; I receive them solely as God’s free gift, for He is rich in grace and mercy. Through my Baptism into the death and resurrection of Jesus, I have been declared righteous and fully forgiven, and I have received the promise of passage through death into everlasting life.

I want my family, my friends, and the entire congregation to know that I place my full confidence and trust—not in my works, my decisions, or even the strength of my faith—but only in my dear Lord and Savior, Jesus Christ, who said: “I am the Resurrection and the Life. He who believes in Me will live, even though he dies” (John 11:25). Therefore, when I die, I know that I will live with Jesus in heaven as I await the resurrection of the body and the life everlasting.

As my final, firm, and public confession of faith and trust in Christ my Lord, I humbly request that the following wishes be considered in planning my Funeral Service and Christian burial:



The Service

I request that the service be planned and conducted by my Pastor(s) at Zion. And I would like for him to use pastoral discretion in all things regarding my funeral (e.g., hymnody, vestments, flower arrangements, music, etc.). I also ask that the service follow the funeral liturgy found in the Lutheran Service Book, according to my Pastor’s arrangement. I ask that my funeral service be held at



Bible Verses

For his sermon, I would like my pastor to consider some of my favorite passages from Holy Scripture:

Add as many Scripture verses as you wish. You may also include a brief note explaining the significance of each one.



Hymns

For the service, I would also like for my Pastor to consider some of my favorite hymns from these hymnals: The Lutheran Service Book (LSB), The Lutheran Hymnal (TLH), The Evangelical Lutheran Hymnary (ELH), or the Te Laudamus Hymnal (TDH).

Add as many hymns as you wish. You may also include a brief note explaining the significance of each one.



Committal

I also request that my pastor or pastors conduct the graveside service, if possible. If they are unable to do so, I ask that they appoint another pastor. I would like the service to follow the Order of Christian Burial in the Lutheran Agenda, according to my Pastor’s arrangement.

Regarding my funeral service, I ask my pastor or pastors and my family to consider the following:
